Patient Diary

The Patient Diary has been designed to help you keep a record of your myeloma appointments, test results, treatments, and any symptoms or side effects that you experience. It also gives you a place to write your thoughts, feelings and questions. This diary can help you keep track of important information related to your myeloma. It also aims to help you and your healthcare team communicate more easily about your myeloma and treatment.

This Patient Diary provides space and prompts to record important information, such as:

  • Healthcare team contact details
  • Support information
  • Appointments
  • Blood test results
  • Treatments and related information
  • Vaccinations
  • Common questions you may want to ask
  • Symptoms and side effects

The Patient Diary is easy to use, with clear sections to record different information. After popular feedback, the Patient Diary now comes in a ring binder format, allowing you to refill each section with more page inserts as you require. Sections of the Patient Diary will be available soon for you to print for free from the Myeloma UK website.
